PATIENT REGISTRATION

Please download the new patient registration packet and complete the forms at home prior to your appointment. Please sign the packet where indicated on pages 4, 5 and 7.

The patient registration packet is a 6-page document that provides Dr. Asgeri with a clear picture of your medical history and current health status. Completing paperwork in advance simplifies the check-in process.

YOUR FIRST VISIT

Patient Check-in: Please plan to arrive 15 minutes prior to your appointment time. Please bring your insurance cards and medication list. After signing in you will be provided a new patient registration packet to fill out. The new patient packet can be downloaded here and filled out prior to your visit. Any applicable co-pay is expected at the time of check-in. Cash, check and credit cards are accepted.T

Waiting Room: The waiting room is equipped with Wi-Fi, digital informational display and entertainment television for your convenience.

Exam Room: The exam rooms are equipped with high tech medical equipment. Interactive digital displays allow you to navigate to the specific information related to your condition or procedure.

Patient Checkout: At the conclusion of your visit with the physician you will make future appointment, receive paperwork and lab orders. The prescriptions are sent electronically to the pharmacy of your choice. You will also schedule endoscopic procedures if required.
Written instructions for the preparation of your procedures will be provided.

INSURANCE AND BILLING

We accept all major insurance plans. Our staff can help file insurance claims with your insurance company to facilitate processing.

Co-pays are due at the time of service. We accept Visa, MasterCard, AMEX, and Discover.

Some procedures require a pre-authorization from your insurance provider. Please contact your insurance company to find out if pre-authorization is required.

Prescription Refills

If you are in need of a prescription refill, please contact your pharmacy and ask them to call us at 760-699-7607 to refill your prescription.

After Hours Emergency: In the event of a life-threatening emergency, call 911 immediately, DO NOT WAIT! Identify yourself as a patient of Oasis Advanced Gastroenterology, and ask the emergency room doctor to contact Dr. Asgeri.

If you find it necessary to contact your doctor after our regular office hours, please call Oasis Advanced Gastroenterology at (760) 699-7607. After hours, your call will be answered by our answering service. Our doctor will promptly return your call.
